Spring Green Art Fest - Artist application

I hereby release and forever discharge the Spring Green Art Fest (SGAF), City of Lynnwood, all sponsoring organizations, their directors, officers, employees, agents, and volunteers from any responsibility, personal liability, claims, loss or damage arising from, or in conjunction with, my participation in the 2016 SGAF. SGAF will not be responsible for any injury sustained by exhibitors or guests or damages done to works of art while within space designated for exhibits. I understand that my artwork is stored at my own risk. Images submitted for the jury are representative of my current work to be exhibited. SGAF reserves the right and discretion to remove any work that does not meet the requirements previously stated. Failure to comply may result in my removal from SGAF, and I may be prohibited from future participation. I agree to allow the SGAF full use of my images for purposes of promoting the Festival this year and in future years. I understand that I am expected to be present in my booth throughout the Festival. By signing this form, I acknowledge having received and read the 2016 Artist Booth Prospectus and agree to abide by all rules and fees/commissions set forth.
